From 1 November 2025, the Australian Government will introduce the Support at Home Program, a new way of delivering aged care services to help older Australians live independently, safely, and comfortably in their own homes. This program is part of the national aged care reform and will replace existing home support systems, creating a simpler, fairer and more flexible approach to care.
Funding is based on an assessment of your needs, made by the Aged Care Single Assessment System (formerly ACAT). This assessment determines what level of funding you will receive – from Level 1 (lowest) to 8 (highest).
Our program provides a wide range of tailored services designed to help you live comfortably and confidently at home. Based on your individual assessment, you may access:
- Clinical care: Nursing, physiotherapy, occupational therapy, and continence support.
- Independence support: Help with showering, dressing, taking medications, and getting to appointments.
- Everyday living services: Assistance with cleaning, gardening, shopping, meal preparation, and respite care.
Services are flexible and can be adjusted at any time to suit your changing needs.
